• A replacement charge of $12 .95 for the cross / $13.95 for the angel

• Shipping/Handling charge of $5.95 (lower 48 US States) - Shipping/Handling Charge to Alaska and Hawaii is $9.95

• Your Name and daytime contact phone number with area code

• The return address where to send the replacement Eternal Light (NO P.O. BOXES, Fedex will only deliver to a physical address)
